For the first interview we prefer to have both parents present. To insure the quality of our conversation, please leave your children at home. You can expect to spend 60 to 90 minutes with us. We will review your child’s history and complete the necessary paperwork, such as the formal consent for the evaluation and appropriate authorizations and releases.

In advance of this appointment please complete the Pediatric Questionnaire and the Acknowledgement of Privacy Practices, which can be found under the downloadable forms on this website.

If your child has had previous assessments, special accommodations, an Individual Education Program (IEP) or 504 Plan at school, please mail/fax us copies of these documents or bring them with you to this appointment.
